We already got the MRD results back yesterday. Check another one off — they came back negative, in the good medical way.

That test was performed after each of the five rounds. As a reminder, Noah went into remission after the first round, though some abnormal cells were still detected. No abnormal cells have been detected since that first test. Again, that doesn’t mean there aren’t any or that he can’t relapse, but it’s a relief each time it comes back clean. Now we’d just like to see his ANC and hemoglobin come up further in next Tuesday’s labs.

The other neat thing yesterday was that Noah got a surprise visit in the middle of a school day. There were many coordinated cheers — and many uncoordinated dance moves from the boys. The amount of support and love from the school staff and classmates has been amazing throughout it all. We love you all — over and out.

MRD (Measurable Residual Disease) Testing

A sensitive test that can detect cancer cells at very low levels (as few as 1 in 10,000 cells for AML). Used to determine if a patient is in remission. The threshold for remission is fewer than 5 abnormal cells per 10,000.

ANC (Absolute Neutrophil Count)

A measure of neutrophils, the white blood cells that fight infection. Normal range is 1,500-8,000. Below 500 is considered severely neutropenic with high infection risk. After chemo, Noah's ANC drops to zero and we wait for it to recover, which can take weeks. Calculated as: WBC × (% neutrophils + % bands) / 100.

Over and Out

During the first round of treatment, Noah and David got walkie-talkies to "super secret communicate" throughout the hospital. The first post after ended with "love you — over and out" and it stuck.

Hemoglobin

The oxygen-carrying protein in red blood cells. Normal range is about 11-16. Noah was admitted with a critically low hemoglobin of 4.7 and receives red blood cell transfusions when it drops to 7 or below.

Remission

When MRD testing shows fewer than 5 abnormal cells per 10,000. This does not mean zero cancer cells or that treatment is over — it means the leukemia is "currently" under control. Noah achieved remission after induction 1 with just 1 abnormal cell per 10,000 detected. None were detected after induction 2.

We also thank Him for supporting Daddy, Mommy, and Micah during this long journey. We know that He will continue His loving care of this dear family and we are grateful.
